Čestné prohlášení k vytvoření tetování
Prohlašuji, že souhlasím s vytvořením trvalého tetování, 24 hodin před tetováním jsem nepožil/a žádné návykové ani omamné látky a rozhodl/a jsem se k tetování dobrovolně. Nemám žádné srdeční potíže, neprodělal/a jsem žloutenku, nejsem nositelem viru HIV, nemám poruchu srážlivosti krve, netrpím cukrovkou ani epilepsií a nejsem těhotná. Nemám žádná fyzická či psychická omezení, která by mohla nové tetování či samotné rozhodnutí o tetování negativně ovlivnit.
Zavazuji se, že budu dodržovat veškeré instrukce týkající se péče o tetování, které jsem obdržel/a od tatérky. Pokud budou nutné korekce způsobené mou nedbalostí, budou provedeny na mé vlastní náklady.
V ceně tetování je jedna korekce (vyjma předělávek (tzv. cover tattoo) a tetování na problematických místech jako jsou hřbety ruky, dlaně, prsty, nárt a chodidla), která by měla být provedena do dvou měsíců po zhojení tetování. Pokud se na korekci nedostavím nebo se tatérovi neozvu v tomto časovém úseku, jsem si vědom/a, že korekci tetování si hradím sám/sama.
Pokud se jedná o předělání staršího tetování – cover tattoo, byl/a jsem obeznámen/a s tím, že jsou nutná nejméně dvě sezení, aby došlo k co nejlepšímu překrytí starého tetování. Dále jsem byl/a seznámen/a s tím, že může docházet k horšímu hojení vzhledem k opakovanému zásahu do již tetované kůže. Korekce u cover tattoo není zahrnuta v ceně.
Výsledná barevnost tetování (sytost barvy a její krycí schopnost) je závislá na typu pokožky a její pigmentaci. Tento fakt nelze ovlivnit. Výsledná barevnost tetování je evidentní zhruba měsíc po jeho dohojení a s tímto faktem jsem obeznámen/a.
Zprošťuji tatérku veškeré odpovědnosti za průběh hojení tetování s tím, že jsem byl/a o správné péči poučen/a v předaném dokumentu „Péče o tetování“ a tatérka nemá možnost správnost hojení více ovlivnit.
Též svým podpisem souhlasím se zveřejněním audiovizuálního záznamu mého tetování na sociálních sítích, webových stránkách a v tiskovinách, které souvisí s propagací tatérů studia.
Toto prohlášení jsem si přečetl/a a svým podpisem souhlasím s výše uvedeným textem, obchodními podmínkami a reklamačním řádem, které jsou přístupné k nahlédnutí na webových stránkách www.tetovanited.cz.
Stvrzuji, že jsem starší 18 let a že jsem dodržel/a veškerá platná opatření MZ ČR.
Tattoo consent form
I declare that I agree to the creation of a permanent tattoo, that I have not consumed any addictive or narcotic substances 24 hours before the tattoo and that I have decided to get a tattoo voluntarily. I have no heart problems, have not had jaundice, am not a carrier of the HIV virus, do not have a blood clotting disorder, do not suffer from diabetes or epilepsy and am not pregnant. I have no physical or mental limitations that could negatively affect the new tattoo or the decision to get a tattoo.
I undertake to follow all instructions regarding the care of the tattoo that I have received from the tattoo artist. If corrections are necessary due to my negligence, they will be made at my own expense.
The price of the tattoo includes one correction (except for retouching (so-called cover tattoos) and tattoos on problematic areas such as the back of the hand, palm, fingers, instep and soles), which should be performed within two months after the tattoo has healed. If I do not show up for the correction or do not contact the tattoo artist within this time period, I am aware that I will pay for the tattoo correction myself.
If it is a retouching of an older tattoo – a cover tattoo, I was informed that at least two sessions are necessary to achieve the best possible coverage of the old tattoo. I was also informed that poorer healing may occur due to repeated intervention in already tattooed skin. Correction for a cover tattoo is not included in the price.
The resulting color of the tattoo (color saturation and its covering ability) depends on the type of skin and its pigmentation. This fact cannot be influenced. The resulting color of the tattoo is evident approximately one month after it has healed and I am aware of this fact.
I release the tattoo artist from all responsibility for the healing process of the tattoo, stating that I was instructed on proper care in the document “Tattoo Care” provided and the tattoo artist has no further influence on the correctness of the healing.
I also agree with the publication of an audiovisual recording of my tattoo on social networks, websites and in printed materials related to the promotion of the studio’s tattoo artists.
